第三章 再生與修復
總論
修復：當組織細胞出現耗損時，身體會被吸收清除，並以實質細胞再生和/或纖維結締組織增生的方式加以修補恢復的過程。
再生性修復
為修復耗損的實質細胞而發生的同種細胞的增生。
分類
生理性再生
皮膚鱗狀上皮，子宮內膜，消化道黏膜，血球。 。 。
不斷更新，維持穩定
病理性再生
在病理狀態下，細胞和組織壞死或缺損後，若損傷程度較輕，損傷週邊的同種細胞再生能力較強，則可由損傷周圍的同種細胞增生、分化，完全恢復原有的結構和功能。

不穩定性細胞
再生能力很強，生理狀態下週期性更換
包括:表皮細胞、黏膜及生殖器官被覆細胞、淋巴及造血細胞等。
穩定性細胞
再生能力較強
生理狀態下，不增殖；病理狀態下，分裂增殖，參與修復
包括:腺體或腺樣器官的實質細胞（肝、胰臟、內分泌腺、腎小管上皮細胞等）、間葉細胞（纖維母細胞、內皮細胞等）。 （分裂增殖能力各不相同）

不具再生能力，出生後即結束細胞週期，永久停止有絲分裂。

瘢痕性修復
不完全修復。在組織細胞無法進行再生性修復的情況下，由損傷局部的間質新生出肉芽組織，後者填補缺損的同時溶解吸收異物，繼之肉芽組織逐漸成熟，轉變為瘢痕組織，使缺損得到修復。
分類
肉芽組織
新生的富含毛細血管和纖維母細胞的幼稚階段的纖維結締組織。
肉眼：表面呈細顆粒狀；鮮紅色；柔軟；濕潤；觸之易出血，無痛覺
鏡下：組織疏鬆（水分多），新生微血管，纖維母細胞，發炎細胞
纖維母細胞：分泌合成膠原纖維/發炎細胞：嗜中性球（化膿菌），淋巴球（病毒感染），巨噬細胞（組織碎片，部分細菌）
新生毛細血管：垂直於創面生長
作用
①抗感染，保護創面 ②填補傷口和其他組織的缺損 ③機化、包裹壞死組織、血栓及其他異物
結局
瘢痕組織
由肉芽組織成熟轉變而來的老化階段的纖維結締組織。
肉眼：收縮狀態，顏色蒼白或灰白，半透明，質硬韌，缺乏彈性
鏡下：大量平行或交錯的膠原纖維束可以發生玻璃樣變性
作用
①填補、連接創口缺損，維持完整性 ②膠原纖維比肉芽組織的抗拉耐力強，堅固性
危害
①瘢痕收縮：關節痙攣和活動受限、幽門阻塞 ②瘢痕沾黏：器官之間或器官與體腔壁之間的沾黏 ③器官硬化：器官內廣泛的纖維化可使此器官質地變硬
創傷癒合
機體遭受外力作用後，皮膚等組織出現斷離或缺損後的癒合過程。包括各種組織的再生和肉芽組織增生，疤痕形成的複雜組合，表現出各種修復過程的協同作用。
依組織損傷程度有無感染
(一) 一期癒合(primary healing)： 缺損小，創緣整齊，無感染，對合嚴密的傷口。 例如：無感染的手術切口
(二) 二期癒合(primary healing)： 缺損大，創緣不整齊，無法整齊對合，或伴隨感染
①壞死組織多，伴隨感染，發炎反應明顯 ②傷口大，收縮明顯，肉芽組織多 ③癒合時間長，形成的瘢痕大
(三) 痂下癒合(healing under scar)：傷口表面的血液、滲出物及壞死組織乾燥後形成硬痂，在其下面進行一期或者二期癒合。

癒合過程
1，血腫形成：出血凝固，輕微炎症，常伴壞死
2.纖維性骨痂形成：機化
3.骨性骨痂形成：分化為類骨和軟骨組織，類骨經由鈣鹽沉積轉變為編織骨，軟骨經由故化過程形成骨組織。
4.骨痂改建或重塑：破骨細胞，成骨細胞協同作用
影響因素
1.骨折斷端及時、正確復位：拔伸、旋轉、折頂、迴旋、端提、捺正、分骨、屈伸
2.骨折斷端及時、牢靠的固定
3.早期運動，保持良好血液供應：關節僵硬、黏連；肌肉萎縮；血栓形成。
影響再生修復的因素
鍛鍊原則 a.全身與局部情況兼顧 b.以恢復患肢的固有生理功能為主 c.功能性鍛鍊以主動式鍛鍊為主，輔以必要的被動式鍛鍊為主 d.運動活動應循序漸進。
